This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.Your Rights
When it comes to your health information, you have certain rights. This section explains your rights and some of our responsibilities to help you. Get an electronic or paper copy of your medical record- You can ask to see or get an electronic or paper copy of your medical record and other health information we have about you. Ask us how to do this.
- We will provide a copy or a summary of your health information, usually within 30 days of your request. We may charge a reasonable, cost-based fee.
- You can ask us to correct health information about you that you think is incorrect or incomplete. Ask us how to do this.
- We may say “no” to your request, but we’ll tell you why in writing within 60 days.
- You can ask us to contact you in a specific way (for example, home or office phone) or to send mail to a different address.
- We will say “yes” to all reasonable requests.
- You can ask us not to use or share certain health information for treatment, payment, or our operations. We are not required to agree to your request, and we may say “no” if it would affect your care.
- If you pay for a service or health care item out-of-pocket in full, you can ask us not to share that information for the purpose of payment or our operations with your health insurer. We will say “yes” unless a law requires us to share that information.
- You can ask for a list (accounting) of the times we’ve shared your health information for six years prior to the date you ask, who we shared it with, and why.
- We will include all the disclosures except for those about treatment, payment, and health care operations, and certain other disclosures (such as any you asked us to make). We’ll provide one accounting a year for free but will charge a reasonable, cost-based fee if you ask for another one within 12 months.
- You can ask for a paper copy of this notice at any time, even if you have agreed to receive the notice electronically. We will provide you with a paper copy promptly.
- If you have given someone medical power of attorney or if someone is your legal guardian, that person can exercise your rights and make choices about your health information.
- We will make sure the person has this authority and can act for you before we take any action.
- You can complain if you feel we have violated your rights by contacting Crystal Brown, Vice President at 800-223-9310.
- You can file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights by sending a letter to 200 Independence Avenue, S.W., Washington, D.C. 20201, calling 1-877-696-6775, or visiting www.hhs.gov/ocr/privacy/hipaa/complaints/.
- We will not retaliate against you for filing a complaint.
Your Choices
For certain health information, you can tell us your choices about what we share. If you have a clear preference for how we share your information in the situations described below, talk to us. Tell us what you want us to do, and we will follow your instructions. In these cases, you have both the right and choice to tell us to:- Share information with your family, close friends, or others involved in your care

- Include your information in a hospital directory
- Marketing purposes
- Sale of your information
- Most sharing of psychotherapy notes
- We may contact you for fundraising efforts, but you can tell us not to contact you again
Our Uses and Disclosures
How do we typically use or share your health information? We typically use or share your health information in the following ways. Treat you We can use your health information and share it with other professionals who are treating you. Example: A doctor treating you for an injury asks another doctor about your overall health condition. Run our organization We can use and share your health information to run our practice, improve your care, and contact you when necessary. Example: We use health information about you to manage your treatment and services. Bill for your services We can use and share your health information to bill and get payment from health plans or other entities. Example: We give information about you to your health insurance plan so it will pay for your services. How else can we use or share your health information? We are allowed or required to share your information in other ways – usually in ways that contribute to the public good, such as public health and research. Help with public health and safety issues We can share health information about you for certain situations such as:- Preventing disease
- Helping with product recalls
- Reporting adverse reactions to medications
- Reporting suspected abuse, neglect, or domestic violence
- Preventing or reducing a serious threat to anyone’s health or safety
- For workers’ compensation claims
- For law enforcement purposes or with a law enforcement official
- With health oversight agencies for activities authorized by law
- For special government functions such as military, national security, and presidential protective services
Our Responsibilities
- We are required by law to maintain the privacy and security of your protected health information.
- We will let you know promptly if a breach occurs that may have compromised the privacy or security of your information.
- We must follow the duties and privacy practices described in this notice and give you a copy of it.
- We will not use or share your information other than as described here unless you tell us we can in writing. If you tell us we can, you may change your mind at any time. Let us know in writing if you change your mind.
Online Privacy Policy
This Privacy Policy is provided to explain the kind of information Austin Medical Products, Inc. may collect while you visit us online, whether through our websites, including www.ampatch.com, or through other websites or online applications, blogs, social networks, or other online or wireless means via computer, mobile device, or other technology (“Austin Medical Products Sites” or “Sites”), and how Austin Medical Products, Inc. maintains and uses that information. By using the Austin Medical Products Sites, you are agreeing to the terms of Austin Medical Products, Inc.’s Privacy Policy. Please be aware that some individual Austin Medical Products Sites may have their own or additional privacy policies and terms of use, which you should review before using such site, including the Terms of Use available on this page. Some of the information we keep on our server is information that you volunteer to provide through forms and some is collected automatically. Unless you volunteer to submit personal data yourself, Austin Medical Products, Inc. will not collect any personal information.
Personal Information You Choose to Provide
In General. While Austin Medical Products, Inc. does not generally require that you provide us with personally identifiable information in order to visit and use the Austin Medical Products Sites, in order to purchase products, we may enable you to provide or post personal information about yourself, including information that could reasonably be used to contact you or to identify you personally, such as first and last name, email address, mailing address, fax number, telephone number, and hospital/facility information (collectively, “Personal Information”). If you choose not to provide Personal Information or other information to us, you may not be able to participate in certain functionalities of the Austin Medical Products Sites. If we combine any demographic or other information we collect about you with Personal Information about you, we will treat the combined information as Personal Information. Individuals Under 13 Years of Age. The Austin Medical Products Sites are open to all individuals. We do not knowingly collect or permit the collection, storage or use of any Personal Information from anyone under 13. If you are a parent or guardian who knows or has discovered that your child under the age of 13 has submitted Personal Information or other information without your permission, upon request, we will promptly: (a) provide direct notice to you indicating what, if any, Personal Information of your child has been collected and how it has been used and/or disclosed; (b) remove your child’s Personal Information or other information from our database, cease the use of such information and direct any other party with access to such information to do the same; and (c) notify you of our compliance with the above. Consent to Processing. The Austin Medical Products Sites are administered from in the United States and the Sites may be replicated to various locations around the world to offer better performance to you. If you are located in the European Union or elsewhere outside of the United States, please be aware that information we collect may be transferred to and processed in the United States. By using the Sites, or providing us with any information, you understand and expressly consent to this transfer, processing and storage of your information in the United States, where privacy laws may not necessarily be as comprehensive as those in your country.
Social Network Applications
Functionality on the Austin Medical Products Sites may permit interactions between the Sites and third-party websites, enabling you to “like,” “share,” or otherwise post content to third-party websites or applications. If you choose to do so, shared information (including any Personal Information) may become public, including being visible on the Sites. The third-party social network websites or applications may allow us to collect and share (or the third party to share) information about you, including Personal Information. Information Automatically Collected and Stored
During your visit, Austin Medical Products, Inc. automatically collects and stores the domain and host from which you access the internet, the internet address of the website from which you linked directly to the Austin Medical Products Sites (if applicable), the date and time you arrived at the Austin Medical Products Sites, how long you spent on the site, which pages you visited, your internet protocol (IP) address, the location of any device used to access the Austin Medical Products Sites, and your computer’s operating system and browser software.
Cookies and Other Tracking Technologies
The technologies used on the Austin Medical Products Sites to collect Use Information, including Device Identification, may include: “Cookies” (which may be html files, Flash files or other technology) are small text files that help store user preferences and activity. Web beacons or similar technologies may be used for a number of purposes, including, without limitation, to count visitors to the Austin Medical Products Sites, to monitor how users navigate the Sites, to count how many sent emails were actually opened or to determine whether content sent was actually viewed. An embedded script is programming code that is designed to collect information about your interactions with the Austin Medical Products Sites, such as the links you click on. These transfers and disclosures may be done with or without notice to you.
Opting Out of Email
If you provide us with your email address, we may send you emails regarding Austin Medical Products, Inc. or the products we offer. You can choose to opt out of receiving email from Austin Medical Products, Inc. at any time. If you wish to opt out with respect to more than one email address, you must provide a separate request for each email address. Every email sent from Austin Medical Products, Inc. will include appropriate unsubscribe instructions. Notice to California Residents
California Privacy Rights: If you are a California resident, you have the right to receive: (a) information identifying any third party(ies) to whom Austin Medical Products, Inc. may have disclosed, within the preceding calendar year, your personal information for that third party’s direct marketing purposes; and (b) a description of the categories of personal information disclosed. Changes to this Privacy Policy
Austin Medical Products, Inc. may at any time update this Privacy Policy. We will provide notice about significant changes by either sending a notice to the primary email address we have on file for you or by placing a prominent notice on the Privacy Policy webpage. Any changes become effective immediately upon posting. Your continued use of the Austin Medical Products Sites after any changes to the Privacy Policy are made will constitute your agreement to be bound by those changes.
